Attached is the petition to put the charter amendment on the ballot. It is two letter size pages.

INSTRUCTIONS FOR CIRCULATING THIS PETITION

Petitioners and circulators must be able to affirm their respective affidavits on the petition page. Especially, note the following:

• If possible, print page two -- the amendment summary and instructions page -- on the back of each petition page. Otherwise, have it on a separate page to circulate with the petition.
• Each page must be circulated by one circulator.
• Petitioners must sign in ink and in the presence of the circulator.
• Petitioners must be registered to vote in Minneapolis and must give their street address where they are registered to vote. Post office boxes are not acceptable.
• Each page must be signed by the circulator, witnessed by a notary public. Notaries public can be found at many financial institutions, legal and accounting offices. If you have trouble finding a notary public, contact the campaign at (612) 850-6897.
